When the rack-and-pinion is in a power-steering system, the rack includes a slightly different design.
Part of the rack contains a cylinder with a piston in the middle. The piston is linked to the rack. There are two liquid ports, one on either side of the piston. Providing higher-pressure fluid to 1 side of the piston forces the piston to go, which in turn techniques the rack, providing the power assist.

There are applications where in fact the gear rack is stationary, as the pinion traverses and others where the pinion rotates on a fixed axis while the gear rack moves.
